Is there a way to format a BD-R or BD-RE without having to burn files? I've played with the UDF2BuildBurn sample, but it seems to want a file tree to write to disk. Is there a way around this to simply format the disk as a blank UDF disk, and then use subsequent import calls to add more content to the disk?

You've mixed three different things together: physical format, logical format and disc appending.

1) Yes, you can use StarBurn_CdvdBurnerGrabber_BluRayFormat(...) API call to format Blu-Ray media.

2) StarBurn is not packet-at-once writing software so you CANNOT do software format and append UDF volume with drag-n-drop later.

3) You can always append UDF volume with StarBurn-based application in "continue session" mode.
